You have a right to request any personal data held by the Council about you by making a subject access request.
This form will take you about 10 minutes to complete, but you can save your progress and come back to it later if you need to.
To complete this form you will need to provide:
- Your name and contact details
- Details of any other people you’re requesting information for
- Two forms of ID for you and anyone else you’re requesting information for (you can upload these now, or opt to provide them later)
- The details of what you’re requesting
